Getting Started
Prior to admission, patients engage in a comprehensive pre-screen assessment facilitated by our dedicated access team through telephone consultation. This initial assessment ensures that each patient’s unique needs are understood and accommodated. Following this evaluation, our access team schedules the patient’s admission at a mutually convenient time, extending transportation assistance if required to ensure a seamless transition into our care.
Upon admission, patients undergo a thorough assessment by our experienced nursing staff, ensuring a holistic understanding of their medical and psychological needs. This assessment serves as the foundation for personalized treatment planning.
Patients will be introduced to their primary clinician and medical provider, who collaborate closely to develop a tailored treatment plan aligned with the patient’s goals and aspirations. Throughout their journey at Wellbridge, patients receive dedicated support and guidance from our multidisciplinary team, fostering an environment conducive to healing and growth.
