Pillars of Patient-Centric Care

Clinician Support
09.17.2021

The concept of patient-centric care seems like a simple one on the surface – however the implementation of such individualized care takes a specific framework – from admission to reintegration into everyday life. It treats a patient as a whole person rather than just their disease.

At Wellbridge, we’ve centered our care on this evidence-supported approach.

What is Patient-Centric Care?

Patient-centric care is when an organization’s values, mission statement and vision reflect patient goals. It allows the patient to be the driver of his/her/their recovery. From an organizational standpoint, it is taking on a holistic approach to each individual and looking at all different aspects of their wellbeing to get a deeper understanding of what led to the use of substances or alcohol.

Leveraging Biopsychosocial Assessments in Treatment

At Wellbridge, our team of social workers leverage the Biopsychosocial Assessment during the admission process. If you aren’t familiar, the assessment is named so because of the different understandings it brings to patient treatment:

  • Bio – the biological understanding such as any medical information that would be relevant, such as physical ailments and genetic factors of vulnerability.
  • Psych – psychological understanding of strengths in behaviors and emotions while also understanding the challenges that an individual can be presented with.
  • Social – understanding the systems of upbringing, family, peers and support, as well as cultural norms such race, gender, sexual orientation and trauma.

At Wellbridge, we use learnings from this assessment to inform different approaches to treatment to support patient needs. We take a three-prong approach to recovery: medical, psychological and wellness care.

Three-Pronged Approach to Patient-Centric Care

At Wellbridge, we use learnings from this assessment to inform different approaches to treatment to support patient needs. We take a three-prong approach to recovery: medical, psychological and wellness care.

Patients receive a treatment plan that outlines their own goals for recovery and act as a blueprint to their treatment at Wellbridge. The care provided by our team is collaborative, including our staff, the patient and their support system, in an effort to work cohesively and meet a patient exactly where they are at.

Overcoming Stigma with an Individualized Approach

Due to the nature of addiction being an isolating disease, and due to the wide-spread misunderstandings about addiction, patient-centric care is a crucial approach to recovery. It focuses on the individual and their specific needs, goals, strengths and challenges that they bring, instead of stigmatizing them, lumping them into a misunderstood disease group.

By leveraging a patient-centric care approach, treatment looks differently for every person that walks through the doors. It enhances and individuals motivations for change while identifying and teaching them to cope with their barriers for recovery. It connects them, as an individual, with appropriate support and resources for continued recovery in their own community.
